Exam Information
Please Read the entire page prior to scheduling
If you are scheduled to see me for an FAA Flight Physical the exam will be at my office at 14300 Sunshine Valley Rd, Piedmont Sd 57769.
Please park in front of the Red cabin.
The charges are as follows:
1st and 2nd class exam Includes urine test $ 230
3rd class exam includes urine test $ 170
EKG if required $ 100
Special Issuance $ 35
Medical issue requiring a CACI $ 25
Faxing Info to FAA $ 10
Exam on off hours (extra fee) $ 30
Waggoner Color Vision (first time exam only) $ 50
Payment: CASH (exact amount appreciated), CHECK or VENMO.
I do NOT take credit cards or debit cards.
Prior to your exam you MUST:
Go to the FAA’s MedXPress website: https://medxpress.faa.gov/ and fill out the application. The form can be completed no more than 60 days before the exam. Complete the application online and submit it. After you submit the application, you will be given an opportunity to download a copy of the application. Please do so. Click on the blue box with “Completed Application” in it.
Print a copy of the application and bring it with you to the exam.
If the FAA Internet, or my Internet is down, if I have the PDF, I can still do the exam.
· Item 17.b. is what I call the FAA's "trick question." 17.b. Do You Ever Use Near Vision Contact Lens(es) While Flying? Yes or No.
You will most likely answer that "No" unless you do wear contact lenses for NEAR vision (readers).
· Section 18, items a. through y. are the “Forever in your life” questions. If you have checked an item “yes” in the past you must again check it “yes.” If this has been previously reported in past applications, in the explanation box, you may put “Previously reported no change.”
· Anything new in Section 18 must be explained.
· Section 19, visits to healthcare professionals, require reporting for the last three years.
o You do not have to report visits to healthcare professionals for routine exams such as flight physicals, annual physicals, insurance physicals, eye exams or dental exams.
o You do have to report visits for any illness or medical condition.
o If there are multiple visits to the same healthcare professional for the same condition, only report the last visit or the day of surgery if there was an operation.

For the exam with me:
· If you need lenses for distant or near vision, be sure to bring them. Do not wear your contacts to the exam.
· Don’t empty your bladder just before you come. I need to check a urine sample when you get here.
· Bring a government issued photo and signature ID such as a driver’s license
